10: Anemia Flashcards
Three H and three nutrients essential to avoid anemia
- EPO, thyroid H, testosterone
2. Fe, B12, Folate
Anemia sx**
Fatigue, weakness, dyspnea, pre-syncope, pale conjunctiva, pale skin
Reticulocyte index >2 means what?
Adequate BM response to anemia
Microcytic, normocytic, and macrocytic values of MCV
Micro: <80
Normo: 80-100
Macro: >100
The earliest and most sensitive index that will change in IDA
RDW
IDA in males/post-menopausal females should prompt an evaluation for what?
GI bleed (esp w weight loss)
3 ways to tx anemia of chronic disease
- Tx underlying disease, dont just give Fe without knowing the cause
- ESA: erythrocytes stimulating factors
- Blood transfusion (for severe anemia, Hb <7
Four ways to tx hemorrhagic shock
- Apply pressure/compression as possible
- Obtain large IV access
- Rapid infusion of 1L NS
- Blood transfusion
A single unit oF RBCs should increase Hb how much?**
1g/dL
Two times to screen gravid woman for Rh Abs
1st prenatal visit + 28 weeks
Two ways to confirm sickle cell dx
- Peripheral smear
2. Hb electrophoresis
Sx and CXR in ACS (acute chest syndrome)
- Sx: CP, dyspnea, cough, hypoxemia, fever
2. CXR: pulmonary infiltrates
Common cause of death in adults with sickle cell
Acute chest syndrome
Cons of autosplenectomy in sickle cell pts
Increased infection with encapsulated organisms - need to be immunized
Fiveways to tx sickle cell
- O2 and breathing exercise with incentive spirometer
- Control pain w NSAIDs, Tylenol, opiates
- Hydroxyurea: increases HbF, so less HbS
- Exchange transfusions
- Stem cell transplant - can be curative but 10% mortality rate
