Lethargy, Weakness, Ataxia, Bizarre Behavior, Seizures, Coma
Differentials for Hypoglycemia:
Liver Failure, Sepsis, Hypoadrenocorticism, Signalment, Neoplasia (Insulinoma, etc), Xylitol Tox, Insulin Overdose, Spurious
BG level considered Hypoglycemia:
BG ≤60
As a part of a hypoglycemia workup, what does AUS allow you to evaluate?
Liver appearance
May visualize insulinoma
As a part of a hypoglycemia workup, what does bile acids allow you to evaluate?
Liver function
As a part of a hypoglycemia workup, what does basal cortisol allow you to evaluate?
Hypoadrenocorticism; can follow up with ACTH Stim test
As a part of a hypoglycemia workup, what does paired Insulin/BG allow you to evaluate?
Insulinoma (High Insulin, low BG is diagnostic)
Treatment for a hypoglycemic crisis:
1mg/kg 50% dextrose IV- diluted
What treatment considerations are needed when a hypoglycemic crisis is due to an insulinoma?
Give dextrose SLOW
IV dexamethasone
Can give glucagon as alternative
What are treatment options for insulinomas:
Diet, Prednisone, Diazoxide, Surgery
What are treatment options for hypoadrenocorticism-induced hypoglycemia?
Prednisone
What are treatment options for signalment-induced (toy breed, juvenile) hypoglycemia?
Diet
What are the diet recommendations for hypoglycemia?
High fat, protein, complex carbs
