1 Flashcards
Why did I let the flight continue when I saw no guidance
CA emphatically assured me that it’d be captured
After guidance didn’t capture why did I allow the captain to continue further
Because the aircraft was not operated in a careless or reckless manner, as to endanger life or property. It was VMC and I decided to call for a missed at 1000’ when we met that gate
What’s CRM
Crew resource management is the utilization of all available resources, information, equipment, and people to achieve safe and efficient flight operations.
4 principles of CRM
Communication
Workload management
Situational awareness
Problem solving and decision making
How did I communicate
Clearly stated we won’t have guidance
Clearly stated we won’t capture laterally
Clearly stated we were too high
Clearly stated we were unstable
How was our workload management
First officers maintain a state of readiness to assume delegated responsibilities, as directed by the captain.
The captain didn’t have sufficient knowledge of our FMS
How was our situational awareness
PM was aware of space and time while on the entire flight
PF dove for runway disregarded stabilized approach criteria
phenomenon called tunneling occurs when task saturated. Tunneling restricts scanning the full range of environmental cues, causing the individual to focus narrowly on what are perceived to be the most salient or threatening cues
How was our problem solving and decision making
It is the responsibility of the first officer to contribute pertinent information to enhance the captain’s decision-making process.
What’s an operational threat
Any threat in the operation that is clear and obvious:
Examples: Aircraft system malfunctions, adverse weather, incorrect paperwork, flight plan or runway change, late arriving passengers.
Strategy: Use the Threat and Error Application Model to order CRM skills and TEM countermeasures to employ effective mitigation strategies.
What’s a latent threat?
Latent Threats (1) Any threat that is not obvious or easily recognized:
Examples: Complacency, distractions, fatigue, personality conflicts/issues.
Strategy: Use the Threat and Error Application Model to order CRM skills and TEM countermeasures to employ effective mitigation strategies.
