02 - congential: aortic stenosis & ASD Flashcards
what are the three most common congenital heart diseases in dogs?
PDA, subaortic stenosis, pulmonic stenosis
most common congenital defects in cats?
septal defects (ASD, ventricular septal defect, endocardial cushion defect) and atrioventricular valvular dysplasia
3 most common defects that cause cyanosis
tetralogy of fallot, right to left shunting PDA, right to left shunting septal defect
(aortic stenosis)
- most often from tissue below aortic valve (subvalvular)
- heritable?
- breeds: newfie, golden, rottweiler, german shephard
- Cx (may be normal)?
- yes
- ex intol, lethargy, syncope (fainting), cough, dyspnea, sudden death
(subaortic stenosis)
- murmur location?
- murmur timing?
- pulse quality?
- left heart base
- systolic
- normal to weak
(aortic stenosis)
echo changes?
(aortic stenosis)
- tall R waves in lead II suggestive of what?
- wide p wave in lead II suggestive of what?
- left ventricular enlargement
- left atrial enlargement
(aortic stenosis)
(radiographs)
normal in dogs with mild disease
3 changes?
enlarged LV/LA, poststenotic dilation or aorta
(aortic stenosis)
- balloon vulvoplasty vs. beta-blockers?
- also usually need to manage what?
- balloon has no benefit to beta-blocker alone
- CHF (diuretics), abx to prevent valvular endocarditis
(aortic stenosis)
- mild?
- moderate?
- severe?
- favorable, rare Cx
- favorable, may get Cx
- Cx, shortened life, risk of sudden death
(Atrail septal defect)
- occurs in dogs or cats?
- typically shunt in what direction?
- both
- L to R
(ASD)
common breeds?
boxer, standard poodle
(ASD)
- Cx?
- may be normal, ex intol, lethargy, syncope, right sided heart failure with large defects (ascites, jugular venous distension, positive hepatojugular reflex)
(ASD)
- murmur is what?
- systolic left basilar murmur
(ASD)
- heart enlargement where?
- RV and RA
