What are the differences between inflammatory and mechanical joint pain?
Inflammatory
- AM stiffness > 1 hour
- fatigue
- better with movement and worse with rest
- systemic symptoms
- steroid responsive
Mechanical
- AM stiffness < 30 minutes
- NO fatigue
- better with rest and worse with movement
- NO systemic Sx
- NOT steroid responsive
What are the red flags of joint pain?
- Fevers, rigors, weight loss
- No comfortable position
- Wakes from sleep
- Loss of function
- Trauma
- Malignancy
In terms of joint parts (synovium, cartilage, etc)... What are each a disease of? Arthritis? Osteoarthritis? Rheumatoid Arthritis? Psoriatic Arthritis? Gout? Pseudogout? Septic arthritis?
- Arthritis is a problem of bone, cartilage,or synovium
- OA is a disease of cartilage
- RA is a disease of synovium
- Psoriatic A (PsA) is a disease of synovium and tendon
- Gout, pseudogout, and septic arthritis: effusion of foreign substance (urate, CPPD, bugs)
