What is encompassed by the definition of rehabilitation?
Active participation of a disabled person and others to reduce the impact of disease and disability on daily life
Involves relearning skills/learning new skills
Why is the impact of a disease very complicated?
Impact of disease has many facets and these are all interlinked and they interact
What ladder structure is used in rehabilitation to identify impact of disease?
Structure –> physiology –learning–> skill –circumstances–> independence
E.g. structure: CNS, muscles, skeleton; physiology: voluntary movement; skill: typing; independence: employment
Define impairment
Signs and symptoms experienced by the individual
Distinguish between activity limitation and participation restriction
Activity limitation: difficulty performing a task
Participation restriction: problem experienced in life situation that impacts a person’s independence
What are the keys activities of daily living?
Mobility Personal hygiene Toilet use Dressing Eating Communication Social skills
What are the key participation restrictions?
Independence in daily living
Family role
Employment
Recreation
NB these all depend on circumstances, e.g. for recreation what hobbies you’re interested in
FIND out what matters most to patient/carers
What things may make someone an ineffective learner?
Poor attitude/outlook Initial personality Other impairments Affected insight/perceptual problem Confidence (too much or too little)
What are the principles of rehabilitation?
Assess impact of dx on person’s life (across 3 domains)
Set goals
Tackle barriers to goals
What are the UMN signs?
Upgoing plantar
Spasiticity/rigidity/increased muscle tone
Paralysis/weakness
Increased reflexes
Define hemiparesis
Weakness of muscles of one side of the body
Degree of movement
Define hemiplegia
Paralysis affecting one side of the body
No movement
Define dysphasia
Language disorder marked by deficiency in the generation of speech or difficulty comprehending language
Define dysarthria
Inability to properly forms words (mechanical issue)
After assessment of the issues a person has, how is rehabilitation carried out?
Tackling barriers the person has at all levels
Impairment - usually medical or nursing treatment
Activities - retain skills, teach alternative skills etc.
Participation - modification of environment, carer support etc.
