Wrist Fracture Screen
bracelet test
Scaphoid fracture screen
Examiner pinches “anatomical snuff box” l Active wrist extension l Active forearm pronation l Thumb and index pinch (“OK sign”)
Scaphoid Fracture test
examiner grips patient’s thumb and
applies pressure in the anatomical
snuffbox over the scaphoid bone
Scaphoid instability tests
watson Scaphoid Test for instability
w forearm and hand in neutral position with very slight pronation
w grasp scaphoid between thumb and index
w passively circumduct wrist
starting in slight extension and ulnar deviation, ending in radial deviation with slight flexion
w positive is a clunk when
scaphoid is releases due to the unstable scaphoid subluxating with movement and clunking back in place when pressure is released
Froment’s sign
Ulnar nerve test,
“Ok” sign indicated ular nercve involvment
normal is a duck beak
tests for carpal tunnel syndrome
phalens versions one and two
tinels
median nerve compression test
Phalen’s Test (1st version)
patient seated • both wrists maximally flexed
with back of hands together
and held in front of the trunk • maintain position for 1 minute
Phalens test ( version 2)
patient seated • examiner gently holds involved
wrist in sustained flexion for
1 minute
median nerve compression test
patient seated l forearm and wrist in neutral position l double thumb compression
just proximal to transverse carpal
ligament for 15 sec – 2 mins l positive test … median nerve
symptoms in hand
tinel
examiner gently taps over carpal
tunnel with fingertips or reflex hammer
test for APL/EPB Tenosynovitis (Intersection Syndrome)
Finkelstein’s Test patient seated or standing patient makes fist with thumb inside the fingers examiner or patient moves wrist into ulnar deviation
