A

A

(what one person thinks is breaking down may be crisis for another)
part 1: list symptoms that are occurring. (irrational responses, headaches, insomnia, isolation)

part 2: makes a plan for what might help when symptoms have worsened. This plan should be very specific and direct. (call Dr. and ask for and listen to directions, arrange to have someone stay with me around the clock)

step 6: crisis planning

A

Meant for use by caregivers on behalf of an individual

step 1: gather info about what person is like when well
step 2: identify symptoms that indicate need for help with self care
step 3: provide names of people who can speak on their behalf
step 4: include health-care providers numbers, med list, allergies, preferred meds and meds they don’t like
step 5: include patients treatment preferences
step 6: include patients preferred treatment facility
part 7: includes preferred alternative facilities and those to be avoided
part 8: includes what identified supports should be doing on their behalf
part 9: contains a list of indicators, developed by patient, that tells supporters they are no longer needed.
