Open wound
A break in the skin
Closed wounds
Injury to tissue without breaking the skin, ( bruising, sprain )
Clean SOHNDS
Not infected ( ideally surgical wounds and intentional wounds created under sterile condition )
Clean contaminated wounds
Urinary, digestive or reproductive system wounds that are inherently non sterile, contain normal flora
Contaminated wound
Unintentional wound not under sterile conditions, high risk for infection
Infected wound
Wound containing a large amount of bacteria and showing signs of infection
Chronic wound
One that does not heal easily in a timely manner, if exposed to friction, pressure or moisture
Partial thickness
Dermis and epidermis layers are broken
Full thickness
Dermis epidermis and subcutaneous tissues are penetrated, muscle and bone may be involved
