KOH/KF Exposure
CRO / FM (or Designee) / Worker
[1] Ensure the following:
[a] The Site ERO has been contacted by one of the following methods:
· Dial 911 or 740.897.2444 on an office phone.
· Dial 740.897.2444 on a cell phone.
· Plant radio (channel 1).
· Activate a fire alarm pull box.
· Send a messenger.
[b] Any individuals that have been exposed on their skin or eyes locate a safety shower in a safe location and use for a minimum of 15 minutes.
· Refer to DUF6-AD-206, First Aid Treatment for Potassium Hydroxide - Job Aid.
[c] For ingestion, move to a safe location.
· Refer to DUF6-AD-206, First Aid Treatment for Potassium Hydroxide - Job Aid.
[d] For inhalation, move to a fresh air location.
· Refer to DUF6-AD-206, First Aid Treatment for Potassium Hydroxide - Job Aid.
FM
[2] Establish communications with the Site ERO.
[3] Assist the Site ERO as the Local Emergency Director (LED).
FM (or designee)
[4] Ensure any personnel assisting with the First Aid Treatment are utilizing the correct personal protective equipment (PPE) has described in the Job Aid.
[5] If exposure resulted from an upset condition as defined by this procedure for a spill/leak/release refer to Section 8.2.1 of this procedure.
[6] Submit an IER or CR and WR as necessary.
HF Exposure
CRO/FM (or Designee)/Worker
[1] Ensure the following:
[a] The Site ERO has been contacted by one of the following methods:
· Dial 911 or 740.897.2444 on an office phone.
· Dial 740.897.2444 on a cell phone.

[d] For inhalation move to a fresh air location.
· Refer to DUF6-AD-201, First Aid Treatment for Hydrofluoric Acid Burns – Job Aid.
FM

CaF2 Exposure
CRO / FM (or Designee)/Worker
[1] Ensure the following:
[a] The Site ERO has been contacted by one of the following methods:
· Dial 911 or 740.897.2444 on an office phone.
· Dial 740.897.2444 on a cell phone.
· Plant radio (channel 1).
· Activate a fire alarm pull box.
· Send a messenger.
[b] Any individuals that have been exposed on their skin or eyes locate a safety shower in a safe location and use for a minimum of 15 minutes.
[c] For ingestion, move to a safe location, rinse mouth and drink water and wait on emergency response personnel.
[d] For inhalation move to a fresh air location and wait on emergency response personnel.
FM
[2] Establish communications with the site ERO.
[3] Assist the Site ERO as the Local Emergency Director (LED).

[4] Submit an IER or CR and WR, as necessary.
UF6/HF Vapor Exposure

[1] Ensure the following:

Radiological Personnel Exposure
CRO / FM (or Designee)/Worker
[1] Ensure the site ERO is contacted for the following:
- Any contamination found on an individual above the neck.
- Breaks in the skin where contamination is suspected.
- Detected or suspected contamination where injury is involved.
- Unsuccessful decontamination of personnel.
[2] By one of the following methods:
- Dial 911 or 740.897.2444 on an office phone.
- Dial 740.897.2444 on a cell phone.
- Plant radio (channel 1).
- Activate a fire alarm pull box.
- Send a messenger.
FM
[3] Establish communications with the Site ERO.
[4] Assist Site ERO as the Local Emergency Director (LED).
FM (or designee)
[5] Ensure HST has been contacted.
[6] Ensure any personnel assisting with the response are utilizing the correct personal protective equipment (PPE) for the given hazard.
[7] Submit an IER or CR and WR, as necessary.
KOH Spill/Leak/Release
Plant Personnel
[1] Inform FM/supervisor of upset condition found.
FM
[2] Request the Control Room to make the following announcement:
ATTENTION ALL DUF6 PERSONNEL
ATTENTION ALL DUF6 PERSONNEL
A Spill of “Potassium Hydroxide” HAS BEEN REPORTED AT “Name Location”. All unnecessary personnel STAY CLEAR of this location. (Repeat announcement)
[3] Establish a safe zone around spill.
Operator Technician/Maintenance Technician
[4] Attempt to stop/isolate the leak remotely if safe to do so.
FM
[5] Contact ES&H for assistance.
[6] Investigate suspected/identified KOH spill as follows.
Note: A minimum of two entrants are required to enter the area.
Entrants
[a] Follow protective action orders given by the FM.
[b] IF authorized by the FM, THEN Setup boundaries and absorbents to isolate the affected area and minimize spreading of the spill.
[c] IF authorized by the FM to investigate the area, THEN Perform actions identifi
[d] Don the following PPE as a minimum:
· Safety goggles and face-shield.
· Tychem QC, Tychem SL, or Quantumwear C coveralls with taped seams, booties, and hood. (Tape areas between hood and respirator and sleeves and gloves to minimize liquid intrusion potential.)
· Chemical resistant shoe covers
· Nitrile inner surgeons gloves
· Outer gloves – 11 mil Nitrile gloves
[e] Maintain communication with outside personnel, IF entrants/standby personnel communication is compromised, THEN Immediately Exit the area.
Note: Doffing assistance should be provided by personnel in PPE.
[7] Clean-up of the spill will be in accordance with guidance provided by the FM and approved work control documents.
FM
[8] Contact the following, as necessary.
- PSS
- ES&H Manager
- WM&T Supervisor
- Operations Manager
- Plant Manager
- SSEP Manager
- ESH&Q Manager
[9] Submit WR, as necessary.
HF Spill/Leak/Release
Worker
[1] Immediately Leave the area AND Report the upset condition to the Site ERO by one of the following methods:
- Dial 911 or 740.897.2444 on an office phone.
- Dial 740.897.2444 on a cell phone.
- Plant radio (channel 1).
- Activate a fire alarm pull box.
- Send a messenger.
DUF6-X-TSR-002, 5.5.2.4

[2] Ensure site ERO has been summoned.
[3] Make the following announcement:
ATTENTION ALL DUF6 PERSONNEL
A HF Spill HAS BEEN REPORTED AT Name Location.
All Unnecessary personnel STAY CLEAR of this location. (Repeat announcement)
DUF6-X-TSR-002, 5.5.2.4
[4] Obtain as much of the following information as possible from the person(s) reporting the spill/leak/release/exposure/injuries.
- Location/Status of leak
- Injuries
- Materials
- Source
[5] IF personnel exposure is reported THEN Refer to Section 8.1.2 of this procedure.
Note: If HF alarms are received, then exit this procedure and refer to appropriate alarm response procedure.
[6] Monitor plant alarms and trends to determine how the leak/spill will affect plant operation, AND Take appropriate actions.
FM
[7] Determine AND Initiate initial protective actions based on the upset condition.
[8] Establish communications with the site ERO.
[9] Respond to the command post, as conditions permit, as the Local Emergency Director (LED) and brief the Incident Commander (IC) on the upset condition.
[10] Assist the IC in ensuring that the Hazardous Material Protection Program requirements for entry into the affected areas include the following, based on site established protocols:
· An evaluation of the need for the establishment of area entry boundaries.
· Acceptable personal protection equipment.
· Personnel and area monitoring.

[11] Submit an IER or CR and WR as necessary.
UF6 Release
Worker
[1] Immediately Leave the area AND Report the upset condition to the Site ERO by one of the following methods:
- Dial 911 or 740.897.2444 on an office phone.

- Plant radio (channel 1).
- Activate a fire alarm pull box.
- Send a messenger.

- Dial 5455 from an office phone.
- Dial 740.289.5455 from a cell phone.
[2] Perform self-monitoring at the nearest Monitoring Area/ PCM Area.
[3] IF the contamination is found or suspected while performing self-monitoring, THEN:
[a] Stay in place and limit physical contact with other personnel.
[b] Contact an HST and wait on assistance.
CRO / FM (or Designee)
[4] Ensure the following:
- Personnel perform self-monitoring.
- Assistance for the HST has been requested.
- IF spill/release/leak is significant and/or has the potential to pose a threat to personnel or the environment, THEN Immediately Contact the site ERO by one of the following methods:
· Dial 911 or 740.897.2444 on an office phone.
· Dial 740.897.2444 on a cell phone.
· Plant radio (channel 1).
· Activate a fire alarm pull box.
· Send a messenger.
[5] Monitor plant alarms and trends to determine how the leak/spill will affect plant operation, AND Take appropriate actions.
[6] Submit an IER or CR and WR, as necessary.
FM
[7] IF exposure resulted from an upset condition as defined by this procedure for a spill/leak/release, THEN Refer to Section 8.1.5 of this procedure.
[8] Determine and Initiate initial protective actions based on the upset condition.
[9] IF emergency response has been summoned, THEN Establish communications with Site ERO AND Report to the command post and brief the IC on the upset condition, as the Local Emergency Director (LED).
Oil/Hydraulic Spills
Worker
[1] Stop the Spill - Take actions to prevent the further spread of spill.
- Place gel cover over storm drain, as necessary.
[2] Warn Others - Report spill to FM, ES&H, and others in the nearby vicinity.
[3] IF any fluid is suspected of or enters a storm drain, THEN IMMEDIATELY Notify the Site ERO by one of the following methods:
· Dial 911 or 740.897.2444 on an office phone.
· Dial 740.897.2444 on a cell phone.
· Plant radio (channel 1).
· Activate a fire alarm pull box.
· Send a messenger.
[4] Isolate and Secure equipment as necessary to stop the leak.
[5] Isolate the spill area - Use physical boundaries, if possible.
[6] Minimize exposure as follows:
- Don necessary PPE.
- Avoid contact with the skin, ingestion, and inhalation.
FM
[7] IF emergency response has been summoned, establish communications with Site ERO, THEN Report to the command post AND Brief the IC on the upset condition, as the Local Emergency Director (LED).
FM (or Designee) / Supervisor
[8] Start and/or Secure building ventilation, as necessary.
[9] Request HST support prior to cleaning up spill.
[10] Contact the following, as necessary:
- PSS
- ES&H
- Waste Management
- Operations Manager
- Plant Manager
- DOE
[11] Submit an IER or CR and WR, as necessary.
Diesel Fuel Spill
Worker
[1] Stop the Spill - Take actions to prevent the further spread of spill.
- Place gel cover over storm drain, as necessary.
[2] Warn others - Report spill to FM, ES&H, and others in the nearby vicinity.
[3] IF any fluid is suspected of or enters a storm drain, THEN IMMEDIATELY Notify the Site ERO by one of the following methods:
· Dial 911 or 740.897.2444 on an office phone.
· Dial 740.897.2444 on a cell phone.
· Plant radio (channel 1).
· Activate a fire alarm pull box.
· Send a messenger.
[4] Isolate the spill area - Use physical boundaries, if possible:
- Isolate the area.
- Ensure ignition sources are secured.
- Ensure smoking is prohibited.
[5] Minimize exposure as follows:
- Don necessary PPE.
- Avoid contact with the skin, ingestion, and inhalation.
FM
[6] IF emergency response has been summoned, THEN Establish communications with Site ERO AND Report to the command post and brief the IC on the upset condition, as the Local Emergency Director (LED).
FM (or Designee) / Supervisor
[7] Start and/or Secure building ventilation, as necessary.
[8] Request HST support prior to cleaning up any spill.
[9] Assist the HST in establishing a “safe zone” around the spill.
[10] Ensure appropriate warning signs are posted.
[11] Contact PSS, ES&H, Plant Management, and DOE to inform them of the following:

[1] Stop the Spill - Take actions to prevent the further spread of spill.
