- Onset:time
- Relationship to travel, season, temperature, drugs, previous treatments, hobbies, pregnancy
- Skin symptoms: 3Ps- pain, pruritis/itch, paraethesia/ prickling
- Constitutional (other systems) symptoms:
- acute: chills, fever, headache
- chronic: fatigue, anorexia, weight loss - Systems review

- Overall patient appearance
- Vital signs
- Four major skin signs: type, shape and margin, arrangement, distribution
- Hair and nail
- Mucous membranes
- General examination
