what are the major features that is used to categorise lesions into LI-RADS categories especially the LR-4 & LR-5 lesions?
these features are based on CECT and MRI studies.
A NEST
A:- ARTERIAL PHASE HYPERENHANCEMENT (APHE)
definition:- lesion shows enhancement greater than the surrounding liver tissue during the arterial phase
significance:- Strong indicator of HCC, especially when combined with other major features.
N: NON-PERIPHERAL “WASHOUT” APPEARANCE
definition:- Lesion becomes hypo enhancing relative to the surrounding liver during the portal venous or delayed phase.
significance:-s/o malignancy ,commonly HCC
E:-ENHANCING “CAPSULE” APPEARANCE
DEFINITION:-Peripheral rim enhancement seen in the portal venous or delayed phases.
SIGNIFICANCE:-indicates a fibrous capsule, characterised of HCC.
S:-SIZE
definition:-diameter of the lesion is measured in centimetres.
significance:-Larger size increases the likelihood of malignancy . The thresholds are:-
<1cm:- generally insufficient to categorise as HCC.
1-2cm:- Requires multiple major features for higher LI-RADS categories.
2cm:- Fewer major features needed to categorise as HCC.
T:-THRESHOLD GROWTH.
definition:-Increase in size by more than 50% within 6 months.
significance:-Rapid growth is indicative of malignancy.
