What does Well’s Criteria assess?
Risk at presentation for DVT diagnosis
What must you include in a Wells score history?
PMH: Bedridden Major sugeries Paralysis/immobilisation History DVT Active cancer
What must you examine in Well’s score?
Pitting oedema Calf swelling 3cm+ Tenderness Collateral veins Swelling of whole leg
What is a good acronym for Well’s score?
CUTEBAPS C – calf swelling >3cm U – unilateral pitting oedema T - tenderness E – entire leg swollen B – bedridden/surgeries A – active cancer P – previous DVT S – superficial veins
How many points suggests likely DVT?
2+
How many points suggests DVT is unlikely?
1 or less
What should you do if DVT is likely?
Proximal leg vein USS with results available within 4 hours if possible.
If not possible within 4 hours:
- D-dimer then
- Interim anticoagulation (apixaban)
- Proximal leg vein USS with results within 24hrs
Offer lifestyle advice
What do you do if DVT is unlikely?
Offer D-dimer with results available within 4 hours. Offer anticoagulation in interim period.
IF D-dimer is positive, offer Proximal leg USS
IF negative, stop anticoagulation and look for alternative causes
