The stroke risk for patients with AF
What does the CHADS2VASC score comprise of?
Congestive heart failure Hypertension Age (75+=2, 65-74=1) Diabetes Sex (female = 1) Stroke Peripheral Vascular Disease
What is a score of 0?
Low risk
What is a score of 1?
Low-Moderate Risk
What is a score of 2+?
Moderate to high risk
What do you do in a CHADS2VASC score of 2+?
Offer anticoagulation on DOAC - Apixaban/riveroxeban (If not tolerated, warfarin)
Offer lifestyle advice
What do you do in a CHADS2VASC score of 1?
Consider anticoagulation.

What advice would you give to a patient starting on warfarin?
Daily INR Tetarogenic Can interfere with other medications Increased bleeding risk Vitamin K (leafy greens) can interact with warfarin
What advice would you give to a patient starting on a NOAC? E.g. apixaban, rivaroxaban
Non-reversible, increased bleeding risk
Contraindicated in: Renal impairment, previous GI bleed
