Tdap stands for
Td (tetanus diptheria) ap (acellular pertussis)
HPV Vaccine
3 doses - start at age 11
up to age 26 for women and men who have sex with men
up to 21 for all men
pneumococcal vaccine
age 65+ (even if had a dose before 65)
smokers
residents of nursing homes
adults who are - diabetic, chronic lung CV liver or renal DZ, nephrotic syndrome, alcoholism, functional or anatomic asplenia
five years after first dose, redose those with chronic renal failure, nephrotic syndrome, functional or anatomic asplenia
varicella zoster vaccine
age 60, all adults
prevent painful rash and post therapeutic neuropathy
influenza vaccine
all people 6 mo or older annual vaccine contraindication= egg allergies 65+ may opt for high dose vaccine nasal vaccine safe for adults unless chronic dz such as asthma
Pap smears
not necessary before 21
every 3 years until 65
30+ may opt to do pap plus HPV screen every 5yrs
not necessary to do pap before starting contraceptives in women less than 21
do not recommend screening for HPV alone or in combination with pap smear before 30
mammograms
begin at age 50
then every two years until 75
gonorrhea/chlamydia
women age 24 and younger and in older women at increased risk of DZ (multiple sexual partners, as well as previous STDs)
HIV
adults 18-65
older adults at increased risk
all pregnant women
colonoscopies
screen for colorectal cancer using fecal occult blood testing, sigmoidoscopy, or colonoscopy, in adults, beginning at age 50 years and continuing until age 75
AGAINST screening age 75-85
PSAs
AGAINST routine screening of PSA for prostate cancer
Purpose of well patient visit
screen for diseases assess rid of future medical problems encourage healthy lifestyle update vaccinations maintain relationship w/health car provider in case of illness
immunizations
patients must accept them
immunizing the unwilling=assault
patients may have fears and require education
Tdap recommendations
one dose of Tdap
Td booster every 10 yr
give Tdap for people who are around infants
give Tdap once per pregnancy (32-36 wks)
update Tdap for puncture wound or laceration unless Td booster was within previous 5 years
