hepatitis B doses
3 - birth, 1-2 months, and 6-18 months
DTaP doses
5- 2 mo, 4 mo, 6 mo, 15-18 mo, and 4-6 years of age
Hib dose
PedVaxHIB has 3 dose series at 2 mo, 4 mo, and booster dose at 12-15 months. ActHIB has 4 dose series at 2 mo, 4 mo, 6 mo, and booster dose at 12-15 months
poliovirus vaccine
IPV SQ or IM in 4 doses- 2 mo, 4 mo, 6-18 months, and 4-6 years
MMR vaccine
2 doses- 12-15 months, 4-6 years of age
contraindication to MMR vaccine
pregnancy, immunodeficient, hematologic or solid tumors, or if patients experience thrombocytopenia within 6 weeks of prior MMR vaccine they should not receive a subsequent dose. Is a Live vaccine
Varicella doses
2 doses SQ or IM at 12-15 months and 4-6 years
contraindications to varicella
pregnant, immonocompromised, previous anaphylactoid reaction to neomycin or gelatin.
what to avoid after varicella vaccine?
avoid salicylates for at least 6 weeks after varicella vaccine d/t concerns over Reye syndrome
pneumococcal vaccine
4 doses- 2 mo, 4 mo, 6 mo, and 12-15 months
meningococcal vaccine
children aged 2-10 yers who have an indication , recommended for all adolescents aged 11-18 years
hepatitis A
if patient is 1-18 years, 2 IM doses at least 6 months apart. if 19 or over, 2 IM doses 6-18 months apart. RECOMMENDED at 12-23 months, second at 18 months (if first was at 12 months)
Influenza vaccine
should be given annually in fall in all children 6 or more months of age. if 6 months- 8 years, may require 2 doses INITIALLY. Give 2 doses if first influenza vaccine. Give 2nd dose 28 days following initial dose

rotavirus
3 doses at 2, 4, 6 or 2 doses at 2 and 4

HPV
Gardasil has 3 doses- time 0, 2, and 6 months of F/U. Cervarix in 3 doses at time 0, 1 and 6 months of F/U. recommended for girls and boys starting at 11-12 years of age
