Dystocia
Long difficult, or abnormal labor. Includes:
Precipitous Labor or birth
Dysfunctional labor
alterations in pelvic structure (passage- pelvic shape)
fetal causes
maternal position
psychologic responses (anxiety= increase cortisol and epinephrine. Relaxes smooth muscle (uterus), increases uteroplacental insufficiency).
Precipitous Labor/birth
any delivery 3 hours or less w/o emergency delivery.
dysfunctional labor
ineffective uterine contractions or maternal bearing down efforts (POWER- this is number one cause )
DEFINITION: abnormal uterine contractions that prevent the normal progress of cervical dilation, effacement, or decent.
* can be hypertonic or hypotonic
RISK FACTORS: body build, uterine abnormalities (ex bicornicate uterus), malpresentation and position of fetus, shape of pelvis and inappropriate timing of medications, LGA babies)
How does the fetus cause dystocia
Passenger- abnormal presentation (breech, macrosomia, LGA (CPD=large baby), number of fetuses
hypertonic dysfunction
is less common. Uterus contracts to much and instead of contracting on top is contracting in the middle so its not pushing the baby down or opening the cervix.
