- SAVE A HAART ▪ Identify delirium 1. acute onset and fluctuating course 2. inattention 3. disorganized thinking 4. altered level of consciousness *must have 1 & 2+ 3 or 4 ▪ Remove any offending agents ▪ Treat delirium

- There are no measurement tools for this
- With proper palliative care a patient and their family can experience peace at death
- We don’t always get it right
- Our job is not done: family supports

- Nothing is a substitute for eyes on a patient
- Use both clinical tools AND assessment for best outcomes
- Palliative care is life-changing, for all involved
