Construct a DTD for reports.
Reports should include a title, page headers and one or more sections. They may
include subsections, footnotes and a references section. Each section and subsection
must start with a header. The references section should be optional. Where it is
included it should be made up of a header followed by one or more references. Each
reference may be structured into author(s), title and date of publication.
