Week 7 - Inpatient Cardiac Rehab Flashcards
Phase 1 cardiac rehab goal
Make sure patient is safe/stable to go home
Intensity
Limit HR increase to +20 if medical tx and +30 if surgical
Or when </= 120 = RPE </= 13
Rest when gets close
What do you use when HR isn’t a reliable indicator
RPE
Time for exercise plan
Begin w 3-5 min bouts
Rest Ratio - 1:2 —> 2:1
Progress to 10-15 min continuous walking
Type of exercise for exercise plan
Endurance: walking or bicycle
Flexibility/stretching
Education: self monitoring
Carotid or radial pulse for HR and rhythm
Borg RPE
Dyspnea scale
Angina scale? NO ANGINA
Healing myocardium - importance of rest and activity
PT intervention post CABG -Bed mobility and transfers
Rolling
Sidelying <> sitting EOB
Sit <> stand
Goals of bed mobility and transfers
Independence w bed mobility and transfers
Upright tolerance/OOB to chair for 2 hours
Airway clearance
Upright positioning
Huff coughing/splinting to cough
Inspiratory muscle training - improved functional capacity and QOL
Goals of airway clearance
Adequate airway clearance to prevent pulmonary complications
Functional mobility and endurance
Ambulation
Stairs
Squat <> stand for low item retrieval
Functional mobility and endurance goals
Independence w all functional mobility
Gradually increase endurance/tolerance to 40-80% HRR
Home: Exercise plan - Symptom recognition and action plan
Nitroglycerin
911
Home: Exercise plan - appropriate activity guidelines
Same type and level of exercise done in hospital
Environmental temp and good air quality
Goal of 20-30 min 1-2x/day by 4-6 wks
Activity interspersed with rest
What temperatures cause the least stress on CVP system
40 to 80 degrees
Patient education and discharge planning
Review sternal precautions
Educate on scar massage
Encourage gradual return to activity
Refer out for outpatient cardiac rehabilitation
