A
  • Symptoms persist more than several days in a row
  • Symptoms that occur > 3 times per month for several months AND are associated with
    diarrhea or constipation
  • Gas is severe/debilitating
  • Sudden increase in pain severity/location
  • Persistent diarrhea, constipation, GI bleeding or unintentional weight loss
  • Persistent even after OTC treatment

Local anesthetics (use and avoidance)

A

Only for topical/external use for itching, irritation, burning, discomfort, pain. Avoid in open sores. Not indicated for relief of bleeding, seepage, prolapse, or thrombosis

Vasoconstrictors (use and avoidance)
Can be used internally and externally for itching discomfort and inflammation. Avoid on open sores. Side effects: may increase BP. May worsen BPH symptoms

Protectants
Reduces itching, dryness and discomfort by acting as a barrier. Most are external or internal application. *Glycerin* is for external use only. (e.g. cocoa butter, mineral oil, shark/cod liver, topical petrolatum)

Corticosteroids
Works as a vasoconstrictor and antipruritic. (e.g. hydrocortisone)

Protect cell surfaces and dries out the affected area to prevent irritation. (e.g. calamine, zinc oxide, witch hazel (external only))

Exclusions for self treating pinworms
1. Liver disease 2. Pregnancy and breastfeeding 3. Under 2 years old unless approved by PCP for OTC treatment 4. Weight under 25 pounds (11kg) unless approved by PCP for OTC treatment 5. Need for repeat dosing (2nd time per OTC label and 3rd time per CDC guidance/clinical practice) 6. Worms other than pinworms 7. Vague symptoms (e.g. negative visual inspection) 8. Severe anal or vaginal itching accompanied by mild abdomina pain, insomnia or irritability 9. +/- tape test

Non-pharmacological therapies for treating pinworms
1. Strict person hygiene (hand washing, nail trimming, abstain from scratching, shower daily, change underwear, nightclothes and sheets daily) 2. Disinfect surroundings (vacuum or mop, avoid sweeping, daily hot water linen washing, 131F to kill eggs, eggs destroyed by UV light so open blinds in the house)
