O: ulnar head: coronoid process of ulna, and the humeral head: medial epicondyle of humerus
I: middle convexity of lateral surface radius

A: pronates and flexes weakly
FCR OINA
O: medial epicondyle
I: base of second metacarpal bone
N: median nerve
A: flexes and ABD (Radially deviates)
Palmaris longus OINA
O: medial epicondyle
I: flexor retinaculum and palmar aponeurosis
N: median nerve
A: flexes hand and tightens aponeurosis
FCU OINA
O: humeral head: medial epicondyle
ulnar head: olecranon and posterior border ulna
I: pisiform, hook hamate and 5th metacarpal
N: ulnar nerve
A: flexes hand and ADD (ulnar deviation)
FDS OINA
O: humero-ulnar: medial epicondyle and coronoid process of ulna
radial head: superior half and anterior border or radius
I: shafts of middle phalanges of medial 4 digits
N: median nerve
A: flex middle phalanges PIP joints, flexes proximal phalanges at MCP joints, flexes hand and wrist
FDP OINA
O: proximal three quarters of the medial and anterior surface of the ulna and interosseous membrane
I: medial: base of distal phalanges of 4 and 5, and lateral part: base of distal, 2 and 3.
N: medial part: ulnar and lateral: ant. interosseous (median nerve)
A: flex hand and wrist, and then flex the DIP, PIP and MCP

O: anterior surface of the radius and interosseous membrane
I: base of the distal phalanx of the thumb
N: anterior interosseous nerve (median N).
A: flex distal phalanx of thumb at IP and flex proximal phalanx thumb at MCP joint
PQ OINA
O: distal quarter of anterior ulna,
I: distal quarter of anterior radius
N: anterior interosseous (median nerve)
A: pronates forearm, deep fibers bind radius and ulna together
