Week 5: Antianginal Drugs Flashcards
What is ventricular wall stress?
Force on fibers tending to pull them apart - we spend energy to oppose this
Contributors to ventricular wall stress?
= (P x r)/2h
P: assoc with pre and afterload
h: vent wall thickness
MOA: NO with angina
activates guanylyl cyclase -> GMP to cGMP -> dephosphorylation of myosin LC -> relaxation
Longer duration nitrates? Uses?
Isosorbide dinitrate or mononitrate. Oral prophylaxis
Nifedipine MOA and TU
Dihydropyridine. HTN, prophylaxis of angina.
What drug class reduces mortality of patients with recent MI?
Beta blockers.
What long term positive effects do beta blockers have?
Decrease mortality and prevent stroke in patients with hypertension.
What is the advantage of combination Nitrates + Beta or Ca Blocker
Mitigate each other’s negative effects for net positive effect.
