Week 5 Flashcards
What does S.M.A.R.T stand for?
S-Specific M-Measurable A-Attainable R-Realistic T-Time limited
What are the steps of the Nursing Process?
- Assessment
- Nursing Diagnosis
- Planning/Goal Setting
- Intervention
- Evaluation
Vital signs include the following:
TPR- temperature, pulse, and respirations.
B/P- Blood Pressure
O2 (oxygen) saturation
Why do we measure Vital Signs?
Vital signs reflect change in function that might otherwise not be observed.
What is the average body temperature?
37 degrees C
What is the normal range for body temperature?
36-38 degrees C
What are the common sites for Temperature Assessment?
Oral, Axillary, Tympanic membrane, and Rectal
What is the normal range for adults when taking the pulse?
60-100 beats/min
What is a pulse?
Palpable bounding of blood flow noted at various points on the body.
Where do we take pulses?
Temporal, Cartoid, Apical, Brachial, Radial, Ulnar, Femoral, Popliteal, Posterior tibial, Dorsalis pedis.
What is bradycardia?
rate < 60
What is Tachycardia?
rate >100
What is the normal range for adults when taking respiration?
12-20 breaths/min.
1 respiration is equal to what?
1 inspiration and 1 expiration (inhaling and exhaling)
What is the normal range if you use the pulse oximetery (SpO2)?
95-100% in room air.
What are the sites to avoid when taking a B/P?
Arm with an IV, PICC line (peripherally inserted central catheter), Dialysis port, Mastectomy (if bilateral use thigh cuff), Surgery, and Amputation.
What is vasodilation?
Blood vessels are widening.
What is vasoconstriction?
Blood vessels are narrowing.
How do we lose heat?
Through radiation, conduction, convection, evaporation, diaphoresis.
Skin regulates body temperature through:?
Insulation, Temperature sensation, Vasoconstriction & Vasodilation.
What are the factors affecting body temperature?
Age, Exercise, Hormones, Circadian Rhythm, Stress, Environment, and Temperature Alterations.
There are three phases of a fever (Pyrexia). What are they?
Phase 1- The Chill Phase
Phase 2- The Plateau
Phase 3- Resolution
What are the factors that influence B/P?
Age, Stress, Ethnicity, Gender, Daily variations, Medications, and Activity, Weight, and Smoking.
