Cholesterol + CVD prevention/progression treatment aims
Treatment aims to:
- decrease progression of atherosclerosis
- decrease risk of MI/stroke in established CV disease or those at high risk of CVD events
- increase overall survival 4
Target lipid levels
Apply to all risk categories
TC <4.0 mmol/L
HDL-C >_ 1.0 mmol/L
LDL-C <2.0mmol/L
Non HDL-C <2.5 mmol/L
TG <2.0 mmol/L
Treatment before drug treatment
Dietary modification
- decrease saturated and trans fats
- increase soluable fibre
- introduce plant sterol enriched milk, margarine, cheese products
- limit alcohol
- decrease weight `
First line therapy
Statins
- block the action of HMG CoA reductase - the rate limiting step in our bodies synthesis of cholestrol
- drugs include
rosuvastatin
fluvastatin
Statins indications and adverse effects
- hypercholesterolaemia
- high risk of coronary health disease +_ hypercholesterolaemia
Myalgia, GI symtpms, headache, sleep disturbane, dizziness
Ezetimibe
- MOA - decrease absorption of cholestrol from diet by inhibiting transport of cholesterol across the intestinal wall
Usually in combination with a statin
Can cause: diarrhoea, myopathy, raised liver enzymes, pancreatitis
