cuffs
seal prevents aspiration of oroparygynal secretions and air leakage between the tube and the trachea. cuffed tubes are often used immdiately after a tracheostomy and re essential when ventilating a tracheostomy client with a mechanical ventilator
children
do not require cuffed tubes because their tracheas are small enough to seal the air space around the tube.
low pressure cuffs
used to distribute a low even pressure against the trachea thus decreasing the risk of tracheal tissue necrosis.
the nurse provides trach care
to maintain patency of the tube and reduce the risk of infection.
initiallly,
the trach may need to be suctioned and cleaned as often every 1 to 2 hours. after inflamm subsides, trach care is done once or twice a day
humidity can be provided
with a mist collar
suctioning is done when
clinet has diff. handeling secretions and suc is to clear passageway.
Though pharynx is not sterile,
use sterile technique to avoid pathgen transfer
suctions caths are either
- open tipped; effective removing thick mucus.
- whistle tipped; less irritating to resp tissues
yankauer device or an oral suction tube
is used to suction the oral cavity
the nurse will determin if suction is needed by assessing the client for
signs of resp distress or evidence tha the client in unable to cough up and expectorate secretions.
accurate nursing judgement is necessary
because suctiong irritates mucous membranes and can increase secretions if performed too frequently.
