Space adequate with tooth loss.. what to do?
Maintain the space if primary posterior teeth are missing.
You do not need to maintain the space if you are missing primary anterior because you won’t lose any space!

If primary tooth is lost before 1/2 to 2/3 of permanent tooth is formed, it will slow the permanent tooth eruption.

So when do you use the space maintenance?
When space is adequate or when space is marginally adequate with extraction, otherwise it is inappropriate.
Treatment options for space maintenance
- Band and Loop
- Distal shoe
- Lingual arches
When do you use Band and loop?
60% successful
Unilateral or bilateral loss of a primary molar.
Permanent molar is easier to bond than occlusally converging primary molar.
When do you use Distal shoe?
60-66% success
Only when there is a missing primary 2nd molar prior to the eruption of the permanent 1st molar.
It is used as a guid for erupting 1st molar
It gets placed 1 - 1.5 mm below the marginal ridge of unerupted 1st permanent molar
when do you use lingual arch type appliances?
55-59% successful
When multiple primary posterior teeth are missing.
Types of Lingual arch type appliances
- LLHA: Fixed removable, when multiple teeth loss in the mandibular arch. has an adjustment loop.
- MxLHA
- TPA (transpalatal arch): can be used when one side of arch is broken. Dont use when teeth are missing on both sides
- Nance: Has a button on the palate for stability. Used when multiple teeth loss on Maxillary arch.
