5 Steps of Sequential Evaluation
- Are they making SGA? (SUBSTANTIAL GAINFUL ACTIVITY)
- Do they have an impairment?
- Does the impairment meet a listing?
- Can they do past work?
- Can they do other work?
4 steps of initial case initiation?
- Review entire file, including any prior filings paper or electronic & copy relevant evidence to current folder.
- Recognize all technical issues & initiate resolution of any potential problems or discrepancies.
- Request clarification or additional information from the claimant or authorized representative.
- Request evidence from medical sources & non-medical sources.
List 3 forms that come in with every new initial claim.
- 3368 - App for disability
- 3367 - contains info regarding prior filings, any FO observations
- 827 - consent to release medical information
What is administrative finality?
A clmnt may have filed before & this means SSA makes a disability determination or decision & it becomes final, the decision will stand as the final word on the matter unless new & material evidence is presented or an obvious gross error becomes evident.
4 methods in which we obtain evidence?
- SSA - approved electronic systems. Megahit HIT or ERE.
- Telephone
- Fax
4 examples of an (AMS) acceptable medical source?
- Licensed physicians (MD or DO)
- Licensed psychologists for the purpose of intellectual disorders only.
- Podiatrists (DPM) for disorders of the foot or ankle.
- Optometrists (OD) for visual disorders only.

Define AOD
Alleged onset date: date the claimant alleges they became unable to work due to medical impairment whether that date appears to be appropriate.
Define POD
Potential onset date: the earliest possible date that the FO (field office) alerts the DDS to consider in order to establish the established onset date (EOD) based on non-medical factors.
Define EOD
Established onset date: the first day the claimant meets the definition of disability & all the relevant entitlement or eligibility factors.
When determining onset, what are the 6 factors we consider?
- Alleged onset date
- Potential onset date
- Claim type
- Successful gainful activity
- Other evidence
- Prior filings
Most favorable EOD for DIB claims is the earliest day all the following factors are met:
- Claimant is not working SGA
- Claimant meets the medical severity of disability
- Claimant is actually insured for benefits
