Zero: at nurses station, figure out what you don’t know. Ensure meds are relevant to pt hx/ condition. Make sure they match MAR.
First: in the med room, gather what you need. Ensure packaging is intact.
Second: at the nurses station. Make sure you have everything you need to administer the medication.
Third: at the bedside, scan barcode, 6 rights, make sure pt knows what they are taking. Ensure pt is safe to take the medication!
High alert medications
Bleeding: heparin, warfarin
Hypotension: digoxin
Hypoglycemia: insulin
Trough:
Minimum blood serum concentration - within 30 min. before next scheduled dose
