CCD/R
Continuity of Care Document or Continuity of Care Record - These are documents developed using the clinical document architecture (CDA) and contain the most common information about patients in a summary XML tag format that can be shared by most computer applications and web browsers. Used mostly in Health Information Exchanges (HIEs) and HIOs.
COLD
Computer Output to Laser Disc - the transmission of reports/data directly into a system without producing a paper copy such as dictated reports.
CPOE
Computerized Provider Order Entry - the entry screens used by practitioners to order tests, medications, etc. Prescription orders interface with the pharmacy formulary and provide reminders and alerts because patient information is also integrated into this function. It can alert the ordering practitioner of other medications that might be contraindicated with one being prescribed or can alert the provider that the drug they are prescribing is not in the facility formulary.
DICOM
Digital Imaging and Communications in Medicine - a standard for the transmission of radiology images
ICD 9/10
International Classification of Diseases 9th Revision, 10th Revision - a diagnostic coding system used throughout the world to capture diagnosis and procedures in numerical (and in case of ICD 10 alpha numeric) codes. These codes are used in billing and assigned to patient diagnoses to determine payment, etc. Used in research and comparing morbidity and mortality statistics in the U.S. and globally.
IEEE1073
A standard that allows medical devices to link/integrate to the EHR database.
LOINC
Logical Observations: Identifiers, Names, and Codes - the standard for the electronic exchange of lab results transmitted to hospitals, payers, clinics, etc. Adopted by all large commercial labs.
NCPDP
National Council for Prescription Drug Programs - Pharmacy related standard for exchange of prescription information in formularies and file transfers.
Networks
PANs - Personal Area Networks designed to link phones, computers, PDA’s in close proximity.
LANs - Local Area Networks links computers in an office, hospital, etc. that are in close promixity.
GANs - Global Area Networks link computers with no geographic limits but is expensive and may result in spotty connections.
WANs - Wide Area Networks can cross city, state, or national borders and are often used to connect LANs together.
VPNs are used to connect someone to an office site that needs access from another location - usually from home using tunneling protocols that encrypt data by the sender and decrypt it at the receivers end via a secure tunnel.
SNOMED
Systematized Nomenclature of Medicine - clinical terminology used in software applications including EHRs - not a coding system as ICD is; however but does have descriptors alpha and numeric for most common patient problems.
XML
Extensible mark-up language - allows exchange data using predefined rules and structure/tags to allow common understanding of the data and tags
