Pharm Goal of Antacids
Provide immediate relief from symptoms
Pharm Goal of Proton Pump Inhibitor (PPI) and H2 receptor blockers
Promote healing of ulcers
Pharm Goal of Triple Drug Therapy
Eradicate helicobacter pylori and prevent recurrence of the disease
MOA and Indication
PPI: binds irreversibly to proton pump in the gastric parietal cells which result in a reduced gastric acid secretion
H2 blockers: blocks h2 receptors in the gastric parietal cells which results in a reduced gastric acid secretion
Indication (PPI+h2 blocker): reduce gastric acid secretion in patient with GERD, gastritis, PUD
Antacids: neutralize gastric acid
Indication: relieve pain and prevent further damage to mucosa in GERD, gastritis, PUD
Side effects of PPI and H2 receptor blocker
Headache
Abdominal pain
Nausea
Diarrhoea
Drowsiness
PPI: Vitamin B12 deficiency
(long term)
H2 receptor blocker: confusion in elderly
side effects of antacids
Calcium & Aluminum :
Constipation, nausea,
stomach cramps, flatulence
Magnesium : Diarrhoea,
nausea, vomiting, abdominal
cramping
Sodium :fluid retention,
hypernatremia
why is PPI preferred
PPI’s low potential for drug-drug interactions is an advantage as the drug can be used in combination with other ulcer medications without having to separate administration timings
simethicone
is an antiflatulent that reduces gas bubbles, antacids containing simethicone decreases the amount of gas associated with bloating and discomfort
why is ppi preferred over h2 receptos blockers
ppi reduce gastric acid secretion to a greater extent than the h2 blockers and have a longer duration of action
why is cimetidine used less frequently than other h2 blockers
because cimetidine inhibits hepatic drug-metabolizing enzymes, resulting in numerous drug drug interactions and must be taken up to 4 times a day
mylanta
is an alternative antacid that can be given if patient feels bloated, simethicone decreases the amount of gas associated with bloating and discomfort
what is triple drug therapy
2 antibiotics + 1 PPI
first line 7-10 days
second line 10-14 days
