Week 13 Flashcards
List at least 4 causes of MODS.
- infection
- surgery
- injury/trauma
- burns
- underlying medical conditions- liver or heart disease
What is the primary treatment consist of when dealing with MODS?
- volume resuscitation
- early antibiotics administration
- early goal directed therapy
- rapid source identification and control
- support of major organ dysfunction
There are 2 major syndromes for haematological dysfunction?
- thrombotic microangiopathy (TMA)
- disseminated intravascular coagulation (DIC)
Define multiple organ failure.
It is the failure of 1 or more organs in the body, it is the most common cause of death in the ICU
Define MODS (multiple organ dysfunction syndrome)
the abnormal cellular responses involving multiple organ systems, it is a sequential process which is closely related to sepsis and sepsis treatment
Define SIRS
Systematic Inflammatory response Syndrome is an inflammatory state affecting the whole body
If SIRS is not treated what can occur?
Sepsis (if not treated) leads to shock (if not treated) which ultimately leads to MODS
what is the treatment of SIRS?
- volume resus
- early antibiotic administration
- rapid source identification and control
- supporting major organ dysfunction
- goal directed therapy
What Thrombotic Microangiopathy? (TMA)
The formation of microvascular platelet platelet aggregates usually caused by injury to microvacular endothelium
What is (DIC) Diseminated intravascular coagulation?
Widespread activation of tissue factor dependant coagulation, it is the insufficient control of coagulation
Wha is the treatment for haematological dysfunction?
-aggressive resuscitation -fluids/blood components/ clotting factors/platelets
What are some of the organ dysfunction scoring systems?
- APACHE
- SAPS
- MPM
What is the SOFA scoring system?
A scoring system which is specific to sepsis related sequential organ failure, as score increases (with dysfunctional organs) mortality increase
What is the treatment for MODS?
- shock management
- need to be aware of secondary insult - problem arise from treatment, Orr condition
- high level of criterial care management - ICU high level nursing/medical care
- prevention and management- timely treatment/ effective shock resus/organ support
What is the early treatment of infection?
Antimicrobial therapy - 5 times mores likely to survive if effective antimicrobial therapy is used
