Week 10 - Wednesday Flashcards
What are the 5 neuropathic tools in the C/S?
Pain Paresthesia SMR’s Ortho test Anything else
What is the strongest clue for C8 nerve root involvement?
Decreased sensation over little finger (high LR)
B list of nerve root problems in the C/S?
(STIFF) Stenosis Tumor Instability Infection Fracture
How would syringomyelia present?
Diffuse cape-like distribution of pain loss and temperature loss over one or both shoulders.
How do corticosteroids affect the spine? What ancillary study can you do to rule out that pathology?
Increase risk of osteoporosis and transverse ligament laxity (C/S instability)
Flexion-extension radiograph
What are the measurements on a flexion/extension radiograph that signify C/S instability?
3.5-4mm of motion of vertebra, 2.5 dens and atlas
What is the sign when a person experienced trauma and is holding their own head? What does it indicate
Rust sign
Instability, fracture, severe ligament damage
If there is nerve damage in the little finger, what are three places of potential nerve damage?
C8 nerve root, brachial plexus, ulnar nerve
Is the ULTT for median nerve more specific or sensitive for nerve root problems?
Sensitive (it could be peripheral nerve or brachial plexus or nerve root)
What are the measurements on a radiograph indicating stenosis?
<12 mm
What is the ddx for lumbar pathoanatomical dx withOUT nerve damage?
Deranged disc Facet Sprain Strain Fracture
What is the pathoanatomical ddx for pelvic?
SI sprain, SI syndrome, SI joint dysfunction
Hip
Pubic symphysis
What percent of LBP originates from the SI joint?
20%
What are 6 ortho tests for the SI?
ASLR
Big 5= Gaenslens, SI compression, SI distraction, thigh thrust, sacral thrust
What is Janda’s Sacroiliac muscle imbalance pattern?
Ipsi piriformis short and tight
Ipsi psoas short and tight
Ipsi glut max weak
Contra glut med weak
