Spondylolysis/listhesis Flashcards
1
Q
Where is the most common location of Spondylolisthesis?
A
L4 over L5
2
Q
What is the difference between Spondylolisis and Spondylolisthesis?
A
Lysis is a pars fracture
Listhesis is a slippage with/without pars fracture
3
Q
What are the signs for Spondylolisthesis
A
- AROM may be normal (extension or hyperextension may aggravate)
- extending from flexed posture may aggravate
- lateral flexion to the side of the fracture may aggravate
- flexion does not aggravate and may even relieve
- hamstring will be in myospasm and psoas might be in spasm
4
Q
How do you know its a spondylolysis based on exam?
A
- lysis = focal tenderness of spine
- deep percussion causes lingering pain
5
Q
What are the neuro findings for spondylolysis?
A
Uncommon although adhesions after healing may irritate nerve root
6
Q
What is the best angle to see a pars fracture?
A
Obliques
7
Q
What advanced imaging is suggested
A
- CT scan
- UWS radiologist recommend MRI because it is good and has no radioation
- SPECT scan (can show hot spots accumulated around fracture if it is active)
8
Q
When is an MRI the first choice regardless where you practice?
A
if you suspect spondylolysis and nerve damage
