Week 1: Safety & Organization Flashcards
Clinical care coordination includes what skills that are necessary to ensure timely and effective patient care?
- clinical decisions
- priority setting
- time management
- evaluation
- use of organizational skills and resources
What 2 skills are required by a nurse in order to engage in complex interactions (list the complex interactions)
2 skills - Leadership & decision making.
Complex interactions that require these 2 skills are:
- collaboration
- negotiation
- conflict resolution
- delegation
It is important to value and practice _____in every nursing interaction.
patient centeredness
To adopt a critical thinking approach to the decision making process, a nurse draws on the following information:
- Applying understanding of patient’s perspectives.
- One’s knowledge.
- Evidence-informed best practice guidelines.
- Experience to the decision-making process.
Nurses must establish _______as these relate to actions taken to meet patients’ needs.
priorities of care
Setting priorities also requires nurses to ______.
know the priority need of each patient
What can create time management challenges for nurses?
- changes in health care delivery
- increasing complexity in all settings of care
What is important for nurses reflect on when considering time management?
Reflect on the impact of technology and other forces and to acquire the ability to set limits and refuse demands that are unreasonable. This way they are managing their time better.
Evaluation is an ____ process.
ongoing
What does the nurse do in the evaluation process?
compares expected outcomes with actual outcomes
Why is it important for the nurse to focus on evaluation of a patient’s progress/outcomes, rather than on a task?
- Lessens chances of distraction.
- Provides patient-centered care.
- Focus shifts from simply completing a task –> how do we achieve recovery or management of symptoms/disease for the patient.
What are these:
RNs “must be accountable and accept responsibility for their own action”
“Organize and prioritize own work, and develop time management skills for meeting responsibilities”
Entry-Level Competencies by BCCNP (British Columbia College of Nursing Professionals)
Define delegation:
Transferring the responsibility to perform a function or intervention to someone who would not otherwise have the authority to perform it (responsibility/intervention is not within the scope of practice or scope of employment for the care provider to whom the care is being delegated)
During delegation who is responsible/ accountable for the function?
The person delegating is accountable for the practice, while the delegate is responsible for performing it safely.
In order to provide quality care to patients, nurses must be organized, set priorities, and what else (3)?
Manage their time
Effectively use resources
Continuously re-evaluate
What is the purpose of organization for nurses?
- increases pt and RN safety
- health promotion
- conserve time & energy
- allows flexibility
What does organization for nurses look like?
- Arrive on time
- Be prepared
- Take notes in the report
- Be flexible
- Create a plan
List the factors that are to be considered when priority setting:
- Patient status (stability of pt & level of care required)
- Patient preferences (involve pt in the plan of care)
- TIme available and workload (#of pts #of tasks)
- Assistance available? (instructor, RN, HCA, Peer)
- Agency routine? (mealtimes? med times? bath day?
- Specific treatment/tests (when? how long? prep needed?)
According to Maslow’s hierarchy, explain the 4 priority levels:
1st priority level: address immediate threats (obstructed airway)
2nd priority level: address actual problems impeding comfort (pain)
3rd priority: address potential problems (assessments, routine VS)
4th priority: future planning (pt education for discharge, improving self-care)
Immediate tasks have a timeline. How long should STAT orders be done in?
~10 mins
Immediate tasks have a timeline. How long should PRN meds be given in?
~15 mins
Immediate tasks have a timeline. How long should a report be given in?
anytime you leave the unit
Some tasks do not require immediate attention such as routine meds, routine treatments, admission protocols. Typically what is the window frame for completing these tasks?
Within 60 mins
What tasks can be done with flexibility and do not require a strict time schedule?
- bathing & hygiene
- daily/q-shift treatments
- documentation
Why should nurses encourage residents to perform as much of their own care as possible?
To optimize their independence & self-confidence, and effectively utilize your own time spent with them.
What are the outcomes for a nurse who constantly evaluates?
- increased flexibility to changes
- improvement in nurse’s ability to manage the unexpected.
What resources to student nurses have in residential care?
- fellow students
- HCA
- instructors
- pt’s involvement
- family’s help
What defines: An ongoing process that provides focus and direction for each phase of the nursing care.
Evaluation
What are the 4 important steps to creating an organizational plan?
- develop a prioritized plan (should include break times)
- time allocation (who needs to be seen first, can you group tasks, estimate, and plan a time per task)
- ensure the plan is realistic (discuss with the instructor + peer, review res. care times with HCA, discuss with res., have flexibility)
- evaluate/analyze the effectiveness of the plan (during and at the end)
How can time be better managed?
By utilizing available resources
