Q

What occurs with Premature withdrawal?

A
  • Problems reintubating, respiratory muscle fatigue, compromised gas exchange
  • REINTUBTION = 8x increase in nosocomial Pneumonia, 6-12 fold increase in mortality risk

Q

What must we see to determine readiness of weaning?

A
  • Evidence of some reveral of underlying cause of failure
  • Oxygenation: P/F ratio >150, FiO2 < .50, PEEP < 8, pH > 7.25
  • Hemodynamics: Stable, not on vasopressors, unless low dose
  • Patient can initiate breaths, follow commands, and can protect airway
