Common indications
A
AF, DVT, Secondary prevention post myocardial infarction

Side effects
A
Increased risk of bleeding, Mild rash, Jaundice, Skin necrosis

Contraindications
A
Pregnancy, Active bleeding, Recent surgery, NSAIDS (increased bleeding risk)

Counselling
A
Maintain a constant intake of vit K (leafy greens), Avoid grapefruit juice and cranberry juice, Importance of adherence, Should be taken at the same time every day, and Limit alcohol intake as it can affect INR, If serious bleeding occurs go to A&E, keep medication in a safe place

Monitoring
A
Usually aim for an INR between 2-3, Initially frequent testing then when stable can increase testing to monthly
