Active bleeding, Severe renal impairment, Hepatic disease, Heart valves, Pregnancy

Counselling
A
Adherence, Bleeding risk, Avoid NSAIDs as they increase bleeding risk, Call 999 or go to A&E if blood in stool or urine, nose bleed lasts longer than 10 minutes or vomiting blood, Make sure to have proper contraception

Monitoring
A
FBC, LFTs, Body weight, Serum creatinine before starting and then annually
