When are vital signs measured?

A
  • On initial contact with client. (eg admission or shift start)
  • During physical assessment.
  • Before and after an invasive diagnostic procedure or surgery.
  • During the administration of medication that affects cardiac, resp, or temp controlling functions.
  • Before, during, and after a blood transfusion.
  • Whenever a client’s condition changes.
  • Whenever an intervention may affect a client’s condition.
  • When a fever or known infection is present (Q4-6 h)
