What are the six vital signs?
A
Temperature, pulse, blood pressure, respirations, oxygen saturation, and pain.

How are decisions about frequency of vital sign assessment made?
A
Nurse collaborates with the health care provider in determining frequency of vital sign assessment and also makes independent decisions regarding their frequency on the basis of the client’s status.

When are vital signs measured?
A
- On initial contact with client. (eg admission or shift start)
- During physical assessment.
- Before and after an invasive diagnostic procedure or surgery.
- During the administration of medication that affects cardiac, resp, or temp controlling functions.
- Before, during, and after a blood transfusion.
- Whenever a client’s condition changes.
- Whenever an intervention may affect a client’s condition.
- When a fever or known infection is present (Q4-6 h)
