First line treatment for herpes virus?
Local measures such as proper fluids, bed rest, proper diet, analgesics and antimicrobial mouthwashes such as chlorhexadine
When and how to prescribe for herpes?
Treat infections in immunocompromised patients and severe in non ICP with aciclovir
1 tablet (200mg) , 5 times daily, for 5 days
What viral patients would you refer?
Immunocompromised and severe infection
Bell’s palsy and infection
How treat herpes labialis?
Aciclovir cream
5% cream in 2g tub
Apply to lesion every 4 hours, five times daily, for 5 days
How treat shingles?
800mg aciclovir
1 tablet 5 times daily
For 7 days
Method of action of aciclovir?
Its a prodrug so first it needs to convert to its active form
- aciclovir monophosphate
Then converted to aciclovir triphosphate
This competes and binds to the viral DNA
- when incorporated, it lacks chemical groups to carry on DNA synthesis, so the viral DNA is terminated
