• Name, address, telephone number, qualification and signature of prescriber
• Medicine Name, quantity, dose and administration instructions
• Date
• Warnings (if relevant- withdrawal period)
• Prescribed under the cascade (where appropriate)
• For Vet Sch 2 & 3 CDs (item prescribed for an animal or herd under the care of the veterinarian)
• For Vet Sch 2 & 3 CDs must include RCVS number of prescriber
• standardised forms are not required for vet Rx‘s

What informations should Vet Rx‘s Labels contain?

• Name and address of the owner
• Name and address of the pharmacy
• Name of the prescribing Vet surgeon
• Identification and species of the animal
• Date of supply
• Expiry Date of the product
• Medicine (Name, quantity, dose and administration instructions)
• Any necessary Warnings
• ‚For animal treatment only‘
• ‚Keep out of reach of children‘

Record keeping - What information is required for Receipt and supply of
POM-V and POM - VPS?

• Name and address of supplier or recipient
• Medicine Name & Quantity
• Batch Number
• Date of supply or receipt
• If written prescription - Name and address of prescriber
• Records and Document must be kept for 5 years
