vestibular treatment Flashcards
treatments for BPPV
habituation techniques/exercises, particle repositioning maneuvers and surgical intervention in rare cases
habituation techniques/exercises aim to …
disperse displaced otoconia within the involved SCC without moving the otolithic out of the canal
-the patient is moved into proactive positions several times a day until symptoms are no longer present
2 examples of habituation techniques
brandt daroff exercises and cawthorne cooksey exercises
particle repositioning maneuvers aim to ….
not just disperse otoconia but to move it out of the canal and back into the utricle where it is probably dissolved by endolymph
3 examples of particle repositioning maneuvers
epley, semont and lemper (BBQ roll)
epley maneuver
used for anterior and posterior canal canalithiasis
epley maneuver steps
starting with the involved side, turn the head in that direction so impacted side is down, lay the patient down then turn the head over to face the other way, have the patient lay on their side, then kick the feet out and have them sit up
-hold each position for at least 30 seconds
semont
used for posterior canal BPPV and for cupulolithiasis
semont steps
starting on the involved side turn the head so the impacted side will be down, rapidly move the patient down to the direction of their impacted side, rapidly move the patient up and all the way to the other side with their head being in the same position and facing down
-hold each position for at least 30 seconds
lemper (BBQ roll)
used for geotropic canalithiasis
lemper (BBQ roll) steps
performed initially on the impacted side and rolling away from it, lay the patient down with the impacted side down, then turn head away and hold that head position, have the patient roll on the side, then on their stomach, other side, then back to center and up
-keep the head steady with the spine throughout the positions
-hold each position for at least 30 seconds
