Vestibular Flashcards
What is the primary diagnostic test for posterior canal BPPV?
Hall pike dix - torsional beat
HPD stands for Head Pitch Down test.
What type of nystagmus is associated with horizontal canal BPPV?
Side-beating nystagmus
How is horizontal canal BPPV assessed?
Supine Roll test
What does it indicate if the nystagmus ‘beat’ is towards the ground?
Geotropic, caused by particles in the canal (canalisthiasis)
Caused by particles in the canal (canalisthiasis).
What does it indicate if the nystagmus ‘beat’ is away from the ground?
Ageotropic, caused by particles stuck to the cupula (cupulolithiasis)
Caused by particles stuck to the cupula (cupulolithiasis).
how do you determine which side is affected with geotrophic nystagmus?
Down beating nystagmus, Stronger response towards the affected side.
ie: left sided affected will have stronger down beat with head turned to left
In the case of left-sided horizontal canalithiasis, what happens when the head is turned to the left?
Stronger geotropic response
how do you determine which side is affected with Ageotrophic nystagmus?
Stronger response (upwards beat) towards the affected side.
Ie: right sided cupulalithiasis will have a stronger Upwards beat when turning head to the left
If turning the head to the right produces strong upward beating nystagmus, what condition is indicated?
Cupulolithiasis affecting the left horizontal canal
How is geotropic nystagmus treated?
Gufani maneuver
In the Gufani maneuver for Right horiz BPPV canal canalisthiasis (geotropic nystagmus). how would you perform the gufani?
lie to the left (good ear) - 60sec, turn head to the ground ( 2min)
In the Gufani maneuver for ageotropic nystagmus, which ear do you lie towards?
Affected Ear, then turn head away
What does the phrase ‘The greatest intensity nystagmus beats towards the affected ear’ imply?
Nystagmus intensity is strongest towards the affected side
