Venulitis with palpable purpura (raised bruises)
- most commonly HSN angiitis
- serum sickness
- RMSF (likes upper extremity)
- cancers
Vasculitis
Inflammation of vessels that result in ischemia
- primary and secondary
- often results in multi organ and skin dysfunction
- cryoglobinemia
- C\P ANCA (C ANCA Wegeners; P in everything else)
Palpable purpura on LUX, systemic symptoms (fever malaise)
- drug induced vasculitis
- allopurinol, gold, thiazides, sulfas, phenytoin, PCN
Palpable purpura (butt and legs), arthritis, GI, Kidney diseas more common in kids
Henoch-Shonlein purpura
- usually self limiting
- may cause intusseception
HSN vasculitis following Hep C infxn, with immune complex deposits
- cryoglobinemia
Painful oral ulcerations, plus 2: recurrent genital lesions, eye, skin, positive pathergy test
Bechets syndrome
- typically self resolving

Vasculitis NOT involving skin: kidney, peripheral neuropathy, bowel disease. Fever weightloss, abdominal, MSk pain
- polyarteritis nodosa
- no lung involvement
- lesions all different ages
PAN identifying features
- aneurysms
- pulmonary arteries NOT involved
- venules not involved
- ANCA not involved
- kidneys most commonly involved: nephrotic syndrome
Pain, paresthesis, and sensory loss in PAN
- mononeuritis multiplex
- necrotizing neuritis of vaso nervorum
Gangrene, livedo reticularis, urticaria and purpura
- PAN skin manifestation
Treatment for most of the vasculitises
- high dose prednisone and cyclophosphamide
Pulmonary capillaries and glomerulonephritis, diffuse alviolar, P-ANCA positive, hemorrhage complication:
More common than PAN
Microscopic polyangiitis
Eosinophilic rhinitis and asthma with vasculitis
- Churg-Strauss
- confused for allergies
- trt: with prednisone and cyclophosphamide
- high mortality if its not treated
renal, upper and lower resp lesions, saddle nose deformity, C-ANCA
Wegeners (granulomatous polyangiitis) - Often includes occular lesions - mononeuritis multiplex - Trt: with prednisone and cyclophosphamide
