vascular trauma Flashcards
2 major causes of vascular trauma
- blunt trauma
2. penetrating trauma
3 classes of vascular traumas
- rupture 4 - repair NOW
- grade 3 - delayed repair
- just a nick 1- no repair
2 general approaches
- surgical - less common now
2. endovascular - better
what is opposing side to bleeds
not perfusing
- can co-exist
***4 hard signs of arterial injury
- pulsatile bleeding
- absent distal pulse
- ischemic limb
- no movement or sense - expanding hematoma/shock
what are soft signs of art. injury
- hx of serioud bleeding, but stopped
- decreased distal pulse
- injury to adjacent nerve/art
- ## small hematoma
*****3 injuries known ot be assoc. with vasc. injuries
- elbow dislocation - brachial
- knee dislocation - popliteal
- proximal tib/fib
3 special consideration injuries
- PMHx of peripheral vasc. injury
- shotgun/shrapnel
- crush
when to do surg. if have soft signs
- high risk injury
2. positive CTA
3 keys to good outcome
- high index of suspician
- investigate well - revasc. quickly
- monitor quick;y
2 basics of repair
- removeing clot
2. restoring flow
main way to get clot out
cath
3 options to restore flow
- open
- bypass
- endo
***key to bypass
- use contralateral vein if possibl
3 parts to compartment sysndrome
- high index of suspician
- beware PTs
- sedated
- long operations
- leg inaccesable for exam - do fasciotomy if unsure
when to operate on compartment
- obvious
2. small incision reveals pressure
2 issues when working with ortho
- genrally makes sense to fix vascular first
2. if must do ortho first, consider shunt
