EVAR STEPS:
A
- Map aorta 3d reconstruction and size graft
- Expose bilateral common femoral arteries
- Introduce sheaths and catheters, perform aortogram
- administer heparin
- Place main body of endograft below renals
- place contralateral iliac liimb
- balloon angioplasty sealing zones / joints
- completion angiogram

Open AAA Repair Steps
A
- Midline laparotomy, reflect small bowel to right, transverse colon cephalad
- Dissect duodenum off aorta, exposing renal arteries. ligate left renal vein if necessary
- Dissect distal aorta/proximal iliacs preserving sympathetics
- heparin, furosemide, and mannitol
- clamp iliac arteries, then proximal aorta
- open aneurysm sac opposite IMA, empty of clot, suture lig lumbar arteries
- Sew in graft with monofilament suture
- re-establish blood flow
- Close aneurysm sac and retroperitoneum

CEA Steps
A
- Arterial line for BP monitoring
- Anterior to SCM, retracted laterally, identify internal jugular vein, ligate facial vein
- Expose, preseerving vagus and hypoglossal nerves
- Heparinize, clamp ICA, CCA, ECA
- Arteriotomy in CCA extening to ICA
- endarterectomy, suture tack distally if needed
- sew in patch, back bleed ICA and ECA
- Closse, unclamp ECA, CCA, ICA
- Leave drain in subplatysmal space
