Endocrine Flashcards
1
Q
Partial Thyroidectomy steps
A
- Transverse incision, elevate subplatysmal flaps
- Take upper pole vessels first
- Mobilize lobe anteriorly and medially, staying close to thyroid gland
- identify parathyroids and preserve blood supply
2
Q
Total Thyroidectomy Steps
A
- Collar incision, raise subplatysmal flaps, separate strap muscles
- separate lateral boder of thyroid gland from strap muscles and carotid sheath, divide upper pole vessels
- rotate thyroid anteriorly, identify recurrent laryngeal nere in tracheoesophageal groove
- Divide ligament of berry anterior to passage of nerve into the larynx
- Separate thyroid off trachea
- identify and inspect parathyroid glands, reimplant if necessary
3
Q
Central neck dissection steps:
A
- Dissect out recurrent laryngeal nerves, and remove all fibroadipose tissue between carotid sheaths laterally, hyoid bone superiorly, barchiocephalic vessels inferiorly
4
Q
Laparoscopic Left Adrenalectomy Steps:
A
- Lateral decubitus position, laparoscopic entry
- Mobilize splenic flexure of colon, divide peritoneal attachments of spleen and lienophrenic ligament
- Reflex spleen medially, mobilize pancreatic tail
- Divide inferior phrenic vessels, and central adrenal vein
- Divide inferior and lateral attachments
