ESTENOSIS MITRAL

A
  • reduccion del orificio valvular mitral a menos de 2 cm (N: 4 A 6 CM)
  • Ligera:1.5 a 2 cm/ moderada 1-1.5 cm / severa <1cm
  • es mas frecuente en mujeres
  • 40% relacion mixta (estenosis mas insuficiencia mitral)
  • etiologia: REUMATICA MAS FRECUENTE/ congenita / degenerativa
  • EM + CIA: SINDROME DE LUTEMBACHER
  • CC: hemoptisis / HAS pulmonar ( al ser irreversible el enfermo mejora)/ ronquera por compresion del laringeo recurrente ( signo de Ortner)

causa mas frecuente

A

CF:

fiebre reumatica (criterios dx: mayores y menores)

DX: 2 MAYORES o 1 may y 2 menores

Criterios mayores

  • A rtritis
  • B aile de sambito ( corea de sydenham)
  • C arditis (afeccion valvular +f: mitral)
  • D ermis ( nodulos subcutaneos en sup. flexion)
  • E ritema

criterios menores

  • afectacion clinica: fiebre / artralgias
  • aumento de VCG /PRC
  • ↑ PR
  • cultivo faringeo / antiestreptolisinas
