What is the normal rate of cervical change during the active phase of labor? (i.e. cervical dilation from 6-10 cm).

A

> or equal to 1 cm every 2 hours. If this is not seen in 2 hours then give oxytocin (protraction). If longer than 4 hours with no change then it is arrest (do c-section).

What color is a chylothorax?

A

Milky/white. If the pleural fluid is not milky (i.e. yellow, ect) then another cause of an exudative effusion (Light’s criteria) must be considered (in this case the only other exudative effusion was a TB pleural effusion).

A

Headaches that get worse with leaning forward (suspect in a patient with lung cancer) (question stem had video showing unilateral venous distension on one side of the neck).

A

A

No. Only give prophylactic antibiotics before dental procedures/ect. for prosthetic valves, a prior history of IE, congenital heart disease,ect).

What does a macrocytic anemia with hyposegmented neutrophils in the setting of setting of anemia, thrombocytopenia, and leukopenia suggest?

A

Myelodysplastic syndrome (MDS). Perform a bone marrow biopsy (would show a hypercellular marrow).

What is the next best step for a pregnant woman with pleuritic chest pain, tachycardia, tachypnea, hypoxia, and respiratory alkalosis whose V/Q scan showed a low probability for PE?

A

CT angiogram (if V/Q scan says low probability but there is a high pre-test suspicion for PE then do further imaging such as CTA). (Initially put D-dimer but that has no use in pregnancy).

What is the next best step for vaginal bleeding >20 weeks GA?

A

Transabdominal ultrasound to assess for placental location (placenta previa, ect). (remember that painless vaginal bleeding after 20 weeks is most likely placenta previa).
