What is the definition of schizophreniform disorder?
Schizophreniform disorder is on the schizophenia spectrum.
Breif psychotic disorder: <1 month
Schizophreniform disorder: >1 month but <6 months
Schizophrenia: > 6 months
What is the definition of schizoaffective disorder?
Schizoaffective disorder is characterized by a significant mood episode–depressive or manic–with concurrent psychotic symptoms in addition to psychosis without mood symptoms that lasts for at least 2 weeks. This would possibly present as a patient that is having residual auditory hallucinations following the administration of an SSRI for depression.
How would you treat medication non-adherence in a patient with schizophrenia?
Long acting injectables (LAI) also called Depots, are a good option. They can last for 2-4 weeks and are a good option for patients with schizophrenia that struggle with medication non-compliance. Medication non-adherence is a common cause of relapse and readdmitance in schizophrenia.
What are the common side effects of methylphenidate?
They are decreased appetite, weight loss, and insomnia. MOA is inhibition of DA and NE reputake in the synaptic cleft.
MDMA and Serotonin Syndrome
MDMA aka Ecstasy is also known as 3,4 methylenedioxy-methamphetamine. It is a synthetic amphetamine with hallucinogenic properties. It can cause euphoria, increased sexual desire and empathy. Intoxication may lead to hypertension, tachycardia, hyperthermia, serotonin syndrome and hyponatremia. Coma, seizures and death may also occur.
What are the first line treatments for smoking cessation and when should they be used?
Nicotine replacement therapy.
Buproprion
Varenicline.
Nicotine replacement therapy is good for both short and long term therapy.
Bupropion is a DA and NE reuptake inhibitor mostly used for anti-depressant properties but also works for smoking cessation. It is contraindicated in patients with bulimia as it lowers the seizure threshold.
Varenicline is an alpha 4 beta 2 nicotninic acetelcholine receptor partial agonists that is used for smoking cessation but is also associated with mood changes and suicidality as well as cardiac issues in predisposed patients.
What are the first line treatments for anorexia nervosa?
Nutritional rehabilitation and psychotherapy–CBT are first line. Medically unstable patients may require hospitalization if they have bradycardia, dehydration, cardiac dysrhythmias, electrolyte disturbances such as hypokalemia, hypophosphatemia.
Olanzapine may also be used for severe cases.
